Geriatrics

India is in a phase of demographic transition and the geriatric population is burgeoning. Therefore, there is an urgent need to produce physicians who understand the unique health problems of the elderly, and who possess the clinical competencies to deal with them. Care of these patients is often fragmented among multiple doctors.
There are very few centers offering a MCI recognised MD in Geriatrics training program, CMC Vellore being one of them. By training these doctors, we will increase the pool of trained manpower in the country so as to deliver cost-effective, expert care to the senior citizens of the country in the spirit of Christ.
The department endeavours to provide holistic care to the aging population by promoting effective preventive, curative, rehabilitative, promotive and palliative care. We also strive to achieve excellence in teaching and research.

Outpatient services
Outpatients are seen on Tuesdays and Fridays from 8 am to 5 pm on the ground floor of the Administrative building (600B). We do comprehensive geriatric assessments and refer the patients when required for inputs from other departments including the superspecialties, physiotherapy, occupational therapy, dietary etc. Approximately four hundred outpatients per outpatient clinic were seen and given a rational prescription, prior to the COVID pandemic.
The Memory Clinic runs on every Saturday mornings. This is a referral clinic, mainly drawing patients from Medicine and Neurology, where assessment and management protocols, which are individualized are set in motion. It is a multidisciplinary consultant led clinic.
A Falls and Balance Clinic is a multidisciplinary clinic, which is consultant led, and is a referral clinic runs on every Wednesday afternoons, where the majority of the patients come from the Orthopedic department. These older patients are usually post-operative patients – following a fracture neck or femur/intertrochanteric fractures. Their bone health is looked into.

Inpatient services
We manage over 1200 inpatients per year.
Consultations
We see around 50 inpatient consultations per month. These consultations deal with patients with numerous comorbidities on numerous drugs and patients with cognitive and mobility issues.
Clinical and laboratory services
There are a whole load of ancillary services which back us up in the form of world class laboratories and radiology services. Each of the super specialty departments also have their state of the art equipment and input into our service regularly.

Ongoing clinics

1.Orthopedic peri-op assessment – We liaise with the Department of Orthopedics and assess their patients over the age of 60 years posted for neck of femur and intertrochanteric surgeries, so that the perioperative period is smooth and they have an appropriate rehabilitation schedule. This liaison has been going on from 2018. This liaison is consultant led.
2.Vascular surgery – We also liaise with the Department of Vascular Surgery to rationalize medications and address issues of cognition, mobility and metabolic disorders, which are rampant in their population of patients. This was started in 2019.
3.RUHSA – is a peripheral hospital attached to CMC Vellore. There is a weekly consultant led clinic there, with a monthly visit to the Day Care Centres attached to this health care facility. All the people who attend the Day Care Centre have had a Comprehensive Geriatric Assessment done on them. This was started in 2019.
4.CMC Chittoor Outpatient facility – There are twice weekly Consultant led Clinics held in Chittoor. This is a town which is in the neighbouring state of Andhra Pradesh, about 35 kms away from Vellore. This is presently only functioning as an outpatient facility. This was started in 2019.

 Training

Undergraduate medical training

The Geriatric faculty help the Department of Medicine in taking lectures and bedside clinics for the undergraduates. Following the revamp of the undergraduate curriculum at the centre, we have much more inputs to instill into the undergraduate medical student.

M.D Postgraduate training

We have an ongoing recognized MD (Geriatrics) program, with a total annual intake of three seats. The postgraduate training is mainly a hands-on training with intensive journal clubs, seminars, bedside clinics, daily rounds and a dissertation. At the end of the program, the postgraduate should be able to deliver holistic care to every individual in an ethical way, keeping in mind the financial burden, the vision of the nation and the ethos of the institution.

Post Graduate Fellowship in Geriatric Medicine

We have started a one year postgraduate course in Geriatric Medicine for postgraduates with a degree in Medicine or Family Medicine. This course also endeavors to enable the doctors to comprehensively assess a geriatric patient and treat him or her in the spirit of Christ.

Post Graduate Diploma in Geriatric Medicine

The postgraduate Diploma in Geriatric Medicine was started with a view to educate the doctors at the grass root level to learn the nuances of Geriatrics. As the course evolved, many of the doctors with post graduate degrees were also coming forward for this course, and hence the whole syllabus had to undergo a massive upgradation, which was done in 2020. This course offers a blended program of online modules and assignments, projects and contact programs, and culminates with a summative assessment. It is one of the more popular courses which the Department of Geriatrics runs with help from the Department of Distance Education.

Clinical and Laboratory Training

Cognitive scales, assessment of nutrition, depression, falls and balance assessment, etc. are part and parcel of a comprehensive geriatric assessment and all the faculty and students are well versed with these scales.

CMEs and CNEs

We hold regular CNEs and CMEs for the nurses and Allied Health personnel in and around Vellore to update their skills in geriatrics. In liaison with the CME department, we conduct CMEs for the general practitioners in and around Vellore.
