Gynaecologic Oncology

Cancers of the breast and uterine cervix are the most common cancers among women in India. Gynaecological cancers have been treated at CMC for several decades. However, with subspecialisation, a separate unit has been looking after women’s cancers since December 2010. This unit separated off from Obstetrics & Gynaecology and became a full fledged department in July 2015. Breast cancers can be diagnosed here but are then referred to the breast surgeons (endocrine surgeons). All cancers of the female reproductive tract are managed by the Department of Gynaecologic Oncology.


Out-patient clinics

• Monday and Thursday afternoons in Centenary building (ISSCC) first floor.

• Gynaecology Cancer Screening Clinic on SATURDAY MORNING (outpatient clinic).

At present, the department treats, on average, 10 women with vulvar cancer, 45 patients with cervical cancer, 80 patients with uterine cancer and 120 women with ovarian cancer each year. Recent advances include laparoscopy and hyperthermic intraperitoneal chemotherapy (HIPEC).

The department works in close collaboration with the Departments of Medical Oncology, Radiation Oncology, Radiology, Pathology and Palliative Care. Quite often intra-operative consultations are sought from colorectal surgery, urology, vascular surgery and plastic surgery so that complex operations can be undertaken to remove all gross tumour. The department has kept up with the advancements of technology with endoscopy, sentinel node mapping and hyperthermic intraperitoneal chemotherapy.

The department organises regular home visits, which keeps a check on the patients who default treatment. The designated social worker assists in mobilising finances, psychosexual counselling and scheduling hospital visits and the oncology nurse takes care of wound care, stoma care and patient follow up.

Scope of services
Primary Care 
·         Cervical cancer
-Awareness programs for cervical cancer prevention in schools and colleges of Vellore.
-HPV vaccination to eligible girls in OPD.
·         Ovarian cancer:
-Screening for ovarian cancer in genetically high risk women, who are unwilling for risk reducing salpingo-oopherectomy
-Risk reducing salpingo-oopherectomy in patients , who genetically are at high risk for ovarian cancer
·         Endometrial cancer
-Screening for endometrial cancer in women who are at high risk for endometrial cancer ( obesity, chronic AUB, Lynch syndrome)
-Risk reducing hysterectomy in patients , who genetically are at high risk for endometrial cancer
Secondary care
·         Cervical cancer
-Diagnosis and  treatment of cervical preinvasive neoplasia
·         Vulval cancer
-Diagnosis and treatment of vulval preinvasive neoplasia
·         Vaginal cancer
-Diagnosis and treatment of vaginal preinvasive neoplasia
·         Endometrial cancer
-Diagnosis and treatment of endometrial intraepithelial neoplasia
Tertiary care
·         Cervical cancer
-Diagnosis and surgical treatment of early cervical cancer.
-Referral of advanced cervical cancer to radiation oncology – unit 1 and medical oncology
-Pelvic exenteration for recurrent cervical cancer if patient is eligible for surgery
·         Vaginal cancer
-Diagnosis and surgical treatment of early vaginal cancer.
-Referral of advanced vaginal cancer to radiation oncology – unit 1 and medical oncology
-Pelvic exenteration for vaginal cervical cancer if patient is eligible for surgery
·         Vulval cancer
-Diagnosis and surgical treatment of vulval cancer.
-Referral of advanced vulval cancer to radiation oncology – unit 1 and medical oncology for adjuvant treatment or upfront chemoradiation if surgical resection is not possible.
·         Endometrial cancer
-Diagnosis and treatment of endometrial cancer with open/ laparoscopic/ robotic surgery.
·         Ovarian and fallopian tube cancer.
-Diagnosis and treatment of ovarian/ fallopian tube cancer with upfront or interval surgery after neoadjuvant chemotherapy.
-Patient with advanced cancer will be referred to medical oncology for neoadjuvant chemotherapy or adjuvant chemotherapy or palliative chemotherapy as per patient’s clinical condition.
·         Gestational trophoblastic neoplasia
-Patients with gestational trophoblastic services will be treated with chemotherapy
·         Follow up of patients with treated cancer.
-Patients who have been treated for gynaecologic cancer will be followed up as per schedule drawn up at the end of treatment
·         Second opinion.
-Patients who have had surgery and treatment for gynaecologic cancer in another institution can obtain second opinion in our OPD with regard to further treatment.
·         Multidisciplinary meeting
-The gynaecologic oncology department meets the departments of radiation oncology unit 1 , medical oncology ,  and pathology to discuss treatment plan of a patient in conjunction with the department of radiodiagnosis
Cancer Screening Clinic:
This clinic emphasises the importance of early detection as well as primary prevention. The clinic aims to work in close collaboration with community health departments (CHAD, RUHSA), Family Medicine and Community Nursing.
Survivor Meet:
Every year, the department organises a survivor meet where cancer patients with their spouses are invited for a time of sharing, education and celebration. This meet provides holistic care and upholds the integrity and personhood of the patient.

Training

The department began a two-year fellowship in 2013 and now has a three-year MCh course in Gynaecologic Oncology. Three senior residents are selected each year. Junior residents who have completed MBBS, DGO and MD can apply for non-PG positions and see if they would like to pursue a career in Gynaecologic Oncology.
